Please come to your appointment with all the necessary forms and insurance cards, so that we are able to bill your carrier in a timely and accurate manner.
Co-pays are expected at the time of each visit. We accept all major credit cards.

APPOINTMENTS
Please have your insurance card available as we will need your member ID and Group numbers. If this is an auto claim or workers compensation claim, the claim numbers and adjusters name and phone number will be required to make an appointment. Our number is: (302)239-2800

FIRST VISIT
Please bring the physical therapy prescription from the referring physician if applicable, as well as your insurance cards and completed forms
Each appointment will last approximately 1 hour. Clients receiving shoulder treatment should bring a tank top, sleeveless shirt, or blouse. Those receiving lower extremity care should bring shorts or loose fitting pants
